WHERE HEALTH AND HOME COME TOGETHER

RESIDENTIAL CARE

Our residential level of care services are intended to meet the needs of residents in the earlier stages of their cognitive impairment(s), and require a lower level of care than residents receiving enhanced or memory care services. Residents that receive this level of care require mild-to-moderate care assistance and supportive services compared to our enhanced and memory care services.

ENHANCED CARE

Our enhanced level of care services are intended to meet the needs of residents in the mid-stages of their cognitive impairment(s), with advanced chronic health conditions, and/or require more complex care. Residents that meet the criteria for this level of care will receive services that have been approved by the Vermont Department of Disabilities, Aging, and Independent Living through a variance program that allows the facility to care for residents with higher level of care needs.

MEMORY CARE

Our memory level of care services are intended to meet the needs of residents with moderate-to severe cognitive impairment(s) that requires specialized care strategies and behavioral interventions. This is our highest level of care with comprehensive services that focus on person-centered, research driven, and supportive care strategies to manage the unique and diversified needs of residents in the later stages of their cognitive disease process.

WHAT MAKES US DIFFERENT

Our small, residential, home environment is thoughtfully and strategically designed to provide inclusive, safe, and therapeutic services that allows our residents to receive the care they need without feeling institutionalized. We do this using advanced safety features found in larger facilities without the confinement of designated locked units that separates residents based on the severity of their cognitive impairment(s). Instead, we have created a cognitively-impaired friendly facility with a core focus on nurturing a living environment and individualized support services that unites the strengths, weaknesses, and individuality of our residents so they never experience a sense of isolation or loss of integrity.

To maintain what makes us different, we approach care and support services with an emphasis on person-centered care and behavioral management strategies that tap into the individual histories, interests, talents, and creativity of our residents. With the support of friends and families, we strive to achieve an atmosphere that provides each resident with familiar comforts of home, and services that support their highest level of independence, dignity, and respect.

“There is a fountain of youth: it is your mind, your talents, the creativity you bring to your life and the lives of people you love. When you learn to tap this source, you will truly have defeated age.” - Sophia Loren
